This form is used for students ages 8-18 to notify the student's resident school district that they will be homeschooled for that school year. Once completed, forms can be emailed or mailed to Brenna Fullner in our Student Services Department at our district office located at 9401 Sharon Drive, Everett, WA 98204. This form is not used if your student ages 8-18 will be enrolled in an online school like WA Virtual Academy (WAVA), Insight School of WA or Connections Academy. For those schools, you need to submit a Choice Transfer Request at the top of this page.
