This form is used for proof of address for enrolling a student, or keeping a student enrolled, when the student does not live with their legal parent/guardian. This form is NOT used for families/students that qualify for Unaccompanied Youth or McKinney Vento/Homeless assistance. Full instructions are on the first page of the document.
