This form is used for students ages 2-7 only, that are currently enrolled in school, and will be withdrawn to homeschool. Once completed, forms can be emailed or mailed to Brenna Fullner in our Student Services Department at our district office located at 9401 Sharon Drive, Everett, WA 98204.If a student is under the age of 8 and not yet enrolled in public school, a homeschool form does not need to be filled and submitted. This form is not used if your student ages 2-7 will be enrolled in an online school like WA Virtual Academy (WAVA), Insight School of WA or Connections Academy. For those schools, you need to submit a Choice Transfer Request at the top of this page.
