STEP 2 - Consent to Share Form

More than half of Mukilteo School District students are eligible for:

Free AP and PSAT exams

Free athletics equipment, and more

In addition to school discounts, families may be eligible to receive reduced costs on a variety of services, from internet and utilities to discounts on park and zoo visits.  You can check out the full list of community discounts hereBy filling out the forms in our two-step process, you’re unlocking resources that fuel student success, school spirit, and access for all! 


STEP 1: Complete the Child Nutrition Benefit Application

While all students receive free universal meals, we still need to verify your family income to determine eligibility for additional free and reduced-cost services. To begin the process, please complete the Child Nutrition Benefit Application form via the button above. You can also complete and return a paper version of the form to your school’s front office.

If your family is already “directly certified” by the state, meaning you receive state benefits such as SNAP, or your child is in foster care or McKinney-Vento housing, you can skip Step 1 and proceed to Step 2.

Student eligibility is not dependent on immigration or citizenship status. No identifiable information will be shared with the federal government. 


STEP 2: Complete the Consent to Share form.

We need your permission to share your eligibility for free and reduced fees with school staff who manage programs, like bookkeepers and ASB staff, so they can apply these discounts (example: ASB cards, PSAT/SATs, uniforms, etc.) to your student’s account. To complete the two-step process, please complete the Consent to Share form via the button above. You can also complete and return a paper version of the form to your school’s front office.

Please complete one form for each child attached to your family account. If you do not see all of your children under the same portal login, please get in touch with your school.

Online Consent to Share Form Instructions

1. A parent or guardian must log into the Skyward Family Access portal. Never logged in before? Use the “forgot password” link and enter the email address on file for your student’s school. You should receive an email with your login information.

Make sure you are on the Family Access portal. If you recently registered online, you may need to switch from the New Student Enrollment portal to the Family Access portal by using the drop-down arrow next to the house icon.

Click on the tile named “Consent to Share Eligibility Form.

Note: Once complete, please repeat this process for EVERY student in your family.

2. After clicking the tile, click “Start” to begin the form.

3. Review the information on the form and how it will be used.

Under the Consent to Share Child Nutrition Program Eligibility Information area of the form:

  • Enter the date 
  • Select from the drop-down to either say “Yes” or “No” to the consent to share
  • If selecting “Yes,” pick the MSD Fee Waiver/Reductions program from the drop-down menu

At the bottom, be sure to click “I agree” to enter your electronic signature.

Click “Complete & Review” at the top to go to the final step.

4. Review the form field entries from the previous step. If you need to make a change, click the “Edit” icon. Otherwise, go to the bottom of the step and click the “I confirm” box.

Click the “Submit Form” button at the top.
